The Clinical Nurse Manager is responsible for assisting in the implementation of home health clinical services within the Certified Home Health Agency (CHHA) in a defined area in compliance with applicable laws, regulations and Agency policies. • Works with the Hospital team to assess patient's home-care needs and establishs a discharge plan.
